During this initial conversation, our intake coordinator will ask a few straightforward questions to complete a preliminary needs assessment for your child or loved one.

Next, we schedule an in-person visit at a time that works best for your family, most often in the comfort of your home. This meeting allows your entire family, including the individual receiving care, to be involved in the process. During the visit, we gather more detailed information, walk you through our home health services, and answer any questions you may have.

Our goal is to ensure you feel informed, supported, and confident from the very beginning. At Hampton Care Group, we prioritize clear communication and a thoughtful intake process so your care experience starts smoothly and stress free.

Developing a Custom Care Plan

After the initial assessment, Hampton Care Group develops a Custom Care Plan designed specifically around your loved one’s medical, developmental, and daily care needs. Transparency is a priority, so you will always have a clear understanding of the services we recommend and why they are important.

The Custom Care Plan outlines the proposed care schedule, types of services such as skilled nursing visits, therapies, and personal care, as well as goals focused on improving your child’s comfort, safety, and quality of life. Once the plan is complete, we schedule a follow up meeting, either in person or by phone, to review every detail together.

Your family is an essential part of this process. You have the final say in all care decisions, and we welcome your feedback. We will adjust the care plan as needed until you feel fully confident, informed, and comfortable with the care approach for your child.

Caregiver Matching

Once our intake process is complete and you approve the personalized plan of care, we begin the caregiver matching process with your family’s needs at the center of every decision. Our home health agency does not assign the next available caregiver. Instead, we thoughtfully match your child with a qualified caregiver based on clinical skills, hands-on experience, personality, and your child’s unique preferences and routines.

For example, if your child enjoys music, benefits from specific communication approaches, or has specialized medical or developmental needs, we take those factors into account when selecting a caregiver. This personalized caregiver matching approach helps create consistency, trust, and comfort for both your child and your family.

By taking a holistic view of your child’s care needs, we aim to place a caregiver who truly feels like the right fit in your home. If at any time you have concerns or feel the match is not ideal, simply let us know. We can quickly arrange a replacement caregiver, often within one to two days. Your peace of mind, satisfaction, and confidence in your child’s care are always our top priorities.

Ongoing Care & Supervision

Our support does not stop once care begins. At Hampton Care Group, it is only the beginning. We are committed to ongoing communication, oversight, and quality assurance throughout your child’s care.

Hampton Care Group caregivers maintain detailed daily care notes in the home, allowing you to review exactly what care was provided at any time. This transparency helps families stay informed, confident, and actively involved in their child’s care.

In addition, our supervisory Registered Nurses conduct regular unannounced visits to ensure caregivers are following the approved Custom Care Plan precisely. These RN supervisors evaluate the quality of care, make updates to the care plan when needed, and are always available to address questions, concerns, or changes in your child’s needs.
